Risk (R)

Increased creatinine level × 1.5 or 1GFR decrease > 25%

Injury (I)

Increased creatinine level × 2 or 1GFR decrease > 50%

Failure (F)

Increased creatinine level × 3, 1GFR decrease > 75%, or creatinine level > 4 mg/dL

Loss (L)

Persistent acute renal failure or loss of function for > 4 weeks

End-Stage Kidney Disease (ESKD) (E)

ESKD for > 3 months
