From: To what extent can clinical characteristics be used to distinguish encephalitis from encephalopathy of other causes? Results from a prospective observational study

Encephalopathy

Decreased/altered consciousness (unconsciousness, the patient is hard to keep awake or extensively tiredness) and/or change in cognitive function (bewilderment, disorientation, latency, reduced ability to recall history or data) and/or personality change (aggression, new onset feeling of despair, inadequate/ strange behavior)

Encephalitis

Encephalopathy (=major criterion) for more than > 24 hrs, with no other cause identified and at least 2 of the following:

Objective fever (≥38 °C)

CSF WBC count ≥5 × 106/L

New onset seizures

New onset of focal neurological findings

CT/ MR findings consistent with encephalitis

EEG findings consistent with encephalitis
