  1. * Dissemination was defined as involvement of two or more non-contiguous organ sites [27]. Patients were labelled as ‘pulmonary, disseminated’ in case of presence of concomitant pulmonary and distant extrapulmonary disease, or ‘extrapulmonary, disseminated’ if there was evidence of dissemination without any lung parenchymal involvement.
